What exactly is a CDM/EPC?
A CDM/EPC is Medicare-provided treatment that your GP can arrange for you if you suffer from a chronic or terminal illness. These are put in place to help you manage your condition via allied health services.
The great benefit of the care plan is that you are able to access allied health services such as podiatry at a rebated cost. Your GP can refer you to up to 5 appointments that can be used within a 12-month period.
Since 2005 the term Enhanced Primary Care became obsolete and was replaced with the term Chronic Disease Management. However, the new term never took off and, to this day, they are still referred to as EPCs by many practitioners.
Who is eligible?
A care plan can be provided to anyone who has a chronic condition, which is considered a condition that has been or is likely to be around for 6 months or longer. Conditions include musculoskeletal conditions, diabetes (which would require feet assessment and care), stroke, cardiovascular illness, asthma and cancer to name a few.
